Ontario Health atHome is seeking a Care Coordinator to assess and determine patient care needs, arrange access to community services, and collaborate with patients, families and hospital partners. You will help plan and authorize home care services, monitor progress, and support protected transitions from hospital to home.

Join our Community Float team for a 8:30am-4:30pm schedule on a 6-month assignment, 1.0 FTE, starting salary $44.480/hr to $47.517/hr.
