- Conduct ongoing comprehensive patient assessments
- Develop and evaluate personalized care plans




- Provide medication administration and advanced wound care
- Manage palliative care emergencies proactively
- Educate patients and families about end-of-life processes Requirements:
- Minimum three years’ experience in end-of-life care
- Registration as an RPN with the College of Nurses
- Excellent communication and critical thinking skills
