Durham Community Health Centre (DCHC) is seeking a HART Hub Outreach Worker to deliver client-centered, addiction-focused health services in the community. You will work with an interprofessional team to reach individuals experiencing homelessness or substance use challenges.

This full time, permanent role requires travel between Oshawa and Whitby, with evenings and weekends possible. You will coordinate wraparound supports, document care, and participate in quality improvement and reporting
