Civic Recruitment Limited is seeking a Leaving Care Worker in Peterborough for a 9-month contract. Support young people transitioning to adulthood and ensure they achieve positive outcomes.
In this hybrid role, you will act as a pivotal contact for young people aged 16-25 leaving care. Collaborate with multi-agency partners to coordinate individual plans that foster smooth transitions into education and stable housing. Your aim will be to empower young people, enhancing their life chances and driving change through effective engagement.
Key Responsibilities:
• Coordinate resources from multi-agency partners for secure transitions
• Work directly with young people to achieve strategic outcomes
• Support families and agencies to drive lasting improvements




• Build connections with hard-to-reach young people
• Deliver targeted interventions for youth at elevated risk
Requirements:
• Experience with young people and care leavers
• Proven ability to engage vulnerable individuals creatively
• Driving licence and access to a car
• Social Work England Registration required
• Enhanced DBS clearance for children's and adults' lists
