Peterborough Regional Health Centre (PRHC) seeks a Nurse Navigator to lead survivorship care for cancer patients, coordinating services and transitioning patients back to primary care. The NN develops, implements and evaluates survivorship models in collaboration with oncology and primary care teams.

The role demands robust clinical oncology expertise, program leadership, and robust communication to support patients and families through complex survivorship needs within PRHC.
