Your responsibilities will include managing anticoagulation therapy, conducting clinical assessments, and collaborating on treatment plans for various hematologic conditions. You will play a crucial role in enhancing patient outcomes by improving access to timely care and continuity for patients.Key Responsibilities:
Conduct assessments and manage patients with venous thromboembolism
Provide anticoagulation monitoring and education




Collaborate on individualized care plans with team members
Coordinate outpatient procedures for bleeding disorder patients
Support clinical triage and urgent care needsRequirements:
Registered Nurse (Extended Class) with CNO
BScN or equivalent nursing degree
Minimum two years’ NP experience, preferably in hematology
Advanced clinical knowledge in VTE and inherited disorders
