Engage with individuals in the Criminal Justice system and provide vital support services. Reporting to the Manager of Client Services, this full time position focuses on assessing individual needs and facilitating access to services post-release.

Candidates must demonstrate solid problem-solving skills, build partnerships with community organizations, and effectively liaise with correctional facilities for assessments. You'll play a critical role in reducing recidivism and ensuring continuity of care. Key Responsibilities:
Engage individuals at their initial Criminal Justice system contact
Complete referral and assessment processes for clients




Act as a resource for CMHA staff and service providers
Liaise with correctional facilities before client discharge
Develop protocols to ensure continuity of care Requirements:
Degree in Social Work or Nursing with registration
Solid communication and problem-solving skills
Experience in partnership building with community partners
Valid driver's license and vehicle for agency business
Knowledge of mental health services and support systems Support individuals transitioning from custody to their communities at CMHA Lambton Kent.
