Join VHA Home HealthCare as a part time Registered Practical Nurse (RPN) in the Eastern Region. Support post-hospital clients in Scarborough and Durham with compassionate, client-centred care. We are seeking a dedicated RPN to play a vital role in our Hospital Partnership Program for home health services.

This part time position involves working morning shifts and participating in a bi-weekly weekend rotation. You’ll conduct thorough assessments and provide essential care, including wound management and diabetes support, tailored to each client’s needs. Key Responsibilities:
Assess patient needs and plan care accordingly
Monitor and evaluate care through ongoing assessments
Ensure high-quality patient care delivery
Maintain skilled relationships with clients and families
Document patient interactions and care modifications Requirements:
Active registration with the College of Nurses of Ontario
RPN diploma from an accredited institution
Valid First Aid or CPR Certification
Experience in acute care nursing preferred
