Empower youth with complex needs as a Service Planning Coordinator at Your Support Services Network (YSSN) in Aurora, Ontario. This full-time role focuses on developing and monitoring comprehensive service plans.

At YSSN, we believe in enhancing lives through collaboration and support. As a vital member of our team, you will partner with the Children’s Treatment Network to assist children under 18, as well as young adults up to 21 still in school. You will facilitate integrated service plans and nurture relationships between service providers and families for optimal care.

Key Responsibilities:
• Lead development of Coordinated Service Plans with families
• Support children and youth with multiple special needs




• Collaborate with various sector providers for service integration
• Foster relationships within the Children’s Treatment Network
• Evaluate and monitor service effectiveness and outcomes

Requirements:
• Bachelor’s degree in social work or similar field
• Minimum 3 years related experience in special needs
• Solid team facilitation and leadership skills
• Police clearance and valid driver's license required
• Second language and cultural awareness are assets
