As a Social Worker, you will collaborate with the Director of Care and Business Manager to facilitate incoming resident transitions. Your role includes supporting families during move-ins, assessing individual needs, and participating in the Continuous Quality Improvement (CQI) program to enhance healthcare services.

Key Responsibilities:
• Support residents and families during the move-in process
• Assess residents’ financial and health-related concerns
• Coordinate with residents' Power of Attorney as needed




• Participate in policy evaluation and CQI activities
• Prepare reports for residence management and corporate office

Requirements:
• Bachelor’s or Master’s Degree in social work
• Minimum one year of relevant healthcare experience
• Preferred experience in long-term care settings
• Commitment to resident-centered care principles
• Robust communication and assessment skills
