This role within the interprofessional team is for a Registered Practical Nurse in Transitional Care. You will manage ALC patients, ensuring a patient-centered approach and contributing to customer satisfaction. Key Responsibilities:
• Provide care to ALC patients awaiting community transition
• Evaluate and address patient needs through assessment
• Collaborate within a multidisciplinary team effectively




• Maintain high standards of communication and organization
• Offer teaching and support to patients and families

Requirements:
• Current Certificate of Registration with the College of Nurses
• Experience in inpatient rehabilitation and managing aggressive behaviors
• Excellent physical assessment and computer skills
• Strong critical thinking and problem-solving abilities
• Flexibility to work 12-hour rotating shifts
