Your role includes supporting families during move-ins, assessing individual needs, and participating in the Continuous Quality Improvement (CQI) program to enhance healthcare services. Key Responsibilities:

- Support residents and families during the move-in process
- Assess residents’ financial and health-related concerns
- Coordinate with residents' Power of Attorney as needed
- Participate in policy evaluation and CQI activities
- Prepare reports for residence management and corporate office Requirements:
- Bachelor’s or Master’s Degree in social work
- Minimum one year of relevant healthcare experience
- Preferred experience in long-term care settings
