This full-time permanent role requires a compassionate Outreach Worker to deliver holistic services for clients in need. You will engage with priority populations, offer culturally relevant resources, and ensure efficient access to community services. Key Responsibilities:
• Deliver comprehensive outreach support and case management services
• Provide financial assistance for personal services and essentials
• Coordinate culturally appropriate service plans for clients




• Educate clients on tenant rights and responsibilities
• Advocate for clients and access community resources

Requirements:
• Completion of a 2-year post-Secondary program in Health/Social Services
• At least 1 year of relevant experience in support services
• Strong knowledge of community programs and resources
• Valid driver's license and access to a vehicle
• Availability for flexible hours, including evenings/weekends
