Responsibilities include conducting health assessments, diagnosing conditions, prescribing medications, and creating individualized care plans. You will also engage in health promotion, disease prevention, and collaborate with healthcare professionals to enhance patient care.
Key Responsibilities:
• Conduct comprehensive health assessments and diagnoses
• Manage chronic conditions and prescribe medications




• Order and interpret diagnostic tests accurately
• Develop individualized care plans for patients
• Participate in quality improvement and clinical protocol initiatives
Requirements:
• Registered as a Nurse Practitioner in Ontario
• Strong skills in primary care and patient communication

• Proficiency in electronic medical records
