Key Responsibilities:
• Conduct health assessments and develop care plans
• Diagnose and manage acute and chronic conditions
• Order and analyze diagnostic tests and labs




• Promote health through patient education and counseling
• Mentor and provide leadership to nursing staff
Requirements:
• Current RN(EC) registration with the College of Nurses of Ontario
• Three years of clinical experience in primary care
• Solid clinical assessment and decision-making skills
• Proficient in Electronic Medical Records use
• Valid Ontario Driver's License preferred
