Key Responsibilities:
• Conduct comprehensive health assessments for clients
• Coordinate and evaluate personalized care plans
• Monitor and document changes in client conditions




• Maintain professional relationships with clients and families
• Participate in team meetings and on-call rotations
Requirements:
• Registered with the College of Nurses of Ontario
• RPN diploma from an accredited institution
• First Aid or CPR Certification
• Nursing experience in acute care preferred
• Valid Ontario driver’s license and vehicle
