Services are individualized and guided by each participant's goals and preferences.Key ResponsibilitiesSupport individuals to participate in community-based activities (e.G., recreation, social outings, cultural or faith-based events, shopping, volunteer activities)Provide companionship and social support during community access activitiesAssist with mobility and safe participation in outings as requiredFollow Individual Support Plans (ISPs) developed by the program teamDocument activities, participant engagement, and outcomes accuratelyCommunicate effectively with program coordinators, facility staff, and familiesEnsure participant safety, dignity,



and respect at all timesReport incidents, changes in participant condition, or concerns promptlyQualifications & RequirementsDiploma or certificate in a related field (e.G., Health Care Aide, Community Support Worker, Social Services) OR equivalent experienceMinimum 1 year experience supporting adults with disabilities, seniors, or individuals in continuing care settingsStrong interpersonal and communication skillsAbility to work independently and follow care plansValid Criminal Record Check (with Vulnerable Sector Check)Valid driver's license and access to a reliable vehicle (preferred)Person-centred approach to careEmpathy, patience, and professionalismReliability and punctualityCultural sensitivity and inclusivenessStrong observation and reporting skillsWork includes evenings and weekends based on participant needsTravel within the service area may be requiredCompensationCompetitive hourly rate (based on experience)Mileage reimbursement (where applicable)Ongoing training and career development opportunities
