This position involves working directly with approximately 200 residents, offering longitudinal care and urgent assessments. Collaborate with an interdisciplinary team including Nurse Practitioners and Care Coordinators, all while minimizing travel and administrative responsibilities.
Key Responsibilities:
• Provide full-scope family practice and urgent assessments
• Enroll residents for continuity of care
• Support transitions of care after hospitalizations




• Conduct medication reviews and preventive health assessments
• Collaborate in case discussions and care planning
Requirements:
• Licensed for independent practice in Ontario
• Minimum two years in family medicine or primary care
• Experience with chronic conditions in older adults
• Proficient with EMRs and virtual-care platforms
• Strong communication skills and clinical judgment
