As a permanent, full time pharmacist at UHN, you will provide vital support to patient care. You’ll conduct medication history interviews, reconcile medications, and collaborate with healthcare professionals to resolve drug therapy issues. Your contributions will also include monitoring drug therapy effectiveness and promoting safe medicine practices.

Key Responsibilities:
• Conduct best possible medication history interviews
• Perform medication reconciliation for patient safety
• Monitor drug therapy for adherence to guidelines




• Respond to drug information queries from staff
• Facilitate clinical teaching and research activities

Requirements:
• Bachelor's or Doctor of Pharmacy degree required
• Certificate of competency from the Pharmacy Examination Board
• Active registration with Ontario College of Pharmacists
• Hospital pharmacy practice experience required
• Excellent communication and organizational skills
