In this role, you will engage clients through phone and in-person interactions, developing therapeutic relationships that promote their commitment to treatment plans. Key activities include employing health screening tools and providing short-term transitional support to individuals in need. Key Responsibilities:
Establish therapeutic rapport with clients via calls and face-to-face
Provide transitional support and case management as necessary




Conduct health screenings to inform treatment plans
Record and analyze data from client interactions
Present information to community agencies about Coordinated Access Requirements:
Diploma in Human Services Counseling or related field
Experience managing clients in withdrawal
Familiarity with addictions and community resources
Crisis Intervention training
