The Families First

Program is seeking an experienced case manager to provide intensive support to families transitioning from shelters or facing housing risks. You'll assess family needs, develop personalized support plans, and coordinate services tailored to each family's challenges. This position requires collaboration with various community resources to ensure the success and stability of your clients. Key Responsibilities:
Provide direct support to families in need
Assess family strengths and needs with appropriate tools




Develop and implement housing intake and discharge plans
Facilitate access to community resources for families
Conduct regular home visits for continuous support Requirements:
Bachelor’s in health or social science
Minimum of 3 years in community outreach
Experienced in case management for high-risk populations
Sensitive to diverse cultural backgrounds
Valid driver's license and access to a vehicle Utilize your case management skills and knowledge of community resources to empower families at risk of homelessness.
