As an integral member of Bayshore's Integrated Care Solutions, you will support community clients through compassionate service. This full-time position requires Monday to Friday daytime availability. You’ll work collaboratively with clients and families, assess needs, and implement psychosocial treatment plans tailored to individual needs.

Key Responsibilities:
• Communicate effectively with clients and providers for coordinated services
• Manage and evaluate a caseload efficiently




• Assess client adjustment to emotional and social challenges
• Establish and modify psychosocial treatment plans
• Recommend community resource referrals for clients

Requirements:
• Active member of Ontario College of Social Workers
• Minimum two years of community-based therapy experience
• Strong written and verbal English skills
• Valid driver’s license and access to a vehicle preferred
• Current CPR certification
