Primary Care Alberta is seeking a Community Health Representative to serve the La Crete area in Alberta. You will work with the Low German Mennonite community, bridging gaps between clients, families, health care providers and local organizations.
The role includes promoting healthy living, disease prevention, immunization awareness, chronic disease support, and parenting programs. Travel across La Crete is required; hours are Mon-Fri 8:30–4:45 with occasional evenings/ weekends.
