Provide exceptional in-home care as the Care Team Clinical Lead for VHA Home HealthCare in Toronto. This role focuses on client assessments, care plans, and personal support worker training in a temporary capacity. As an experienced RN/RPN, you will play a pivotal role in creating and monitoring client care plans while ensuring effective support for personal support workers.
