The Supportive Housing Registered Nurse (RN) provides comprehensive and advanced primary health care services, emphasizing health promotion, wellness, harm reduction, health education, and disease prevention in a supportive housing setting for individuals actively and problematically using various substances. Reporting to the Manager of the Overlook, the RN supports the mission, vision, and values of the North End Community Health Centre by delivering relationship-based, client-centered care to diverse populations.
The program supports tenants living in a 60-unit building where all are actively and problematically engaging with various substances. The RN works collaboratively within the multidisciplinary team at the Overlook, which includes BSW, MSW and Health Case Manager, to provide holistic care tailored to individual and community needs. The RN also collaborates with the MAP and Safer Supply teams, as well each patient’s primary care provider, whether they are followed by the NECHC or another provider in community,
The RN utilizes a harm reduction framework and considers the social determinants of health in assessing, planning,
and implementing nursing interventions. Their work includes promoting wellness, managing chronic conditions, preventing disease, and providing education and counseling to empower clients in managing their health. Collaboration across NECHC programs and with community agencies is essential to ensure integrated care and responsiveness to changing trends and needs within the community.
