As a Habilitation Expert, you will be instrumental in creating and executing individualized habilitation plans while supervising a dedicated team of Habilitation Assistants. This role invites you to actively engage in person-centered planning and develop sticky connections with program participants, families, and service coordinators. Your leadership will guide team meetings and ensure the effective tracking of participant progress.

Key Responsibilities

- Create individualized goals reflecting participant strengths
- Oversee implementation of treatment in various settings




- Mentor and manage the Habilitation Assistant team
- Act as a liaison for participant circles of support
- Document progress and service provision in detail

Requirements:
- Bachelor's degree in Human Services or similar
- Minimum one year of related experience required
- Solid communication and organizational abilities
- Skills in operating adaptive and supportive devices
- Valid NYS driver's license preferred
