As a Primary Care Network Clinician in Chilliwack, you will conduct thorough client assessments and facilitate their access to community-based resources and rehabilitation programs. You will also create and implement individualized engagement and treatment plans, working closely with medical professionals and client families for effective service delivery.

Key Responsibilities

- Conduct environmental assessments for clients in need
- Facilitate educational workshops for community resources
- Provide brief therapy sessions and handle case management




- Monitor and document client progress and treatment outcomes
- Advocate for client needs towards service access and support

Requirements:
- Master's degree in Clinical Counselling or Social Work
- Minimum of two years' clinical experience
- Valid B.C. Driver's license and access to a vehicle
- Proficient in assessment tools and crisis intervention techniques
