As a member of an interprofessional team in the patient medical home, the Nurse Practitioner will work collaboratively with physicians and team members to provide high quality, evidence-based primary health care to an assigned patient population of all ages. There will be a primary focus on unattached patients.

Our ideal candidate has:

· Demonstrated experience in successfully working as part of an interdisciplinary team to provide patient and family-centered care in the patient medical home.

· Previous Nurse Practitioner experience in primary health care setting.

· Advanced organizational, prioritization and panel managements skills.

· The ability to take initiative and be a self-starter and self-learner.

· Decision making, strategic and critical thinking skills.





· Strong relationship building skills with the team, patients, and their families.

· Previous experience in using electronic medical records.

· A Master of Nursing with a Family Nurse Practitioner focus.

· Current registration with CARNA.

Pay: $50.39-$66.99 per hour
