Position Summary
Operating within the 20-bed Cheam Supportive Housing Facility, the Services Plan Navigator specializes in intensive case management and aggressive service access support to transition previously unsheltered community members into a reliable, indoor environment. Rooted in the Stó:lō philosophy of lets’emó:t, this role serves as the central point of contact for coordinating care, managing individualized service plans, and systematically dismantling barriers to medical, mental health, substance use, and provincial social systems.
The Services Plan
Navigator ensures no resident slips through the cracks of the regional health and housing continuum
Key Responsibilities
- End-to-End Case Management: Own the full case management cycle for a dedicated caseload, including comprehensive intake, ongoing vulnerability assessments, continuous progress reviews, and transition/discharge planning.
- Proactive Service Access Coordination: Serve as a dedicated navigator to identify, map, and secure timely access to external health and social services—including primary medical care, detox beds, residential substance use treatment, psychiatric care, and legal counsel.
- Systems & Benefits Navigation: Directly assist residents in completing complex applications for financial aid, disability benefits (PWD), housing subsidies, and status registration, removing bureaucratic friction.
- Meticulous Case Documentation: Maintain highly organized, accurate, and confidential electronic case files, care records, and tracking indicators to monitor resident outcomes and ensure seamless data sharing across authorized care teams.
- High-Touch Systems Advocacy: Accompany and advocate for residents navigating complex provincial clinical and social systems, strictly safeguarding their right to culturally safe, trauma-informed treatment.
- Inter-Agency Case Collaboration:
Organize and lead case-conferencing circles with regional partners, including Seabird Island Health, the First Nations Health Authority (FNHA), Fraser Health, and BC Housing (HEARTH framework) to unify resident care paths.
- Land-Based Healing Integration: Culturally anchor service plans by coordinating direct access to local Elders, traditional Stó:lō healing circles, and community-led wellness initiatives.
- Crisis De-escalation & Safety Planning: Apply advanced trauma-informed practices and crisis prevention techniques to stabilize mental health crises or behavioral incidents within the facility.
- Harm Reduction & Overdose Response: Administer life-saving intervention (Naloxone) when necessary, distribute harm reduction supplies, and coordinate closely with the Cheam Mobile Overdose Prevention Services (MOPS) van.
- Independent Living Transition Coaching: Support residents in building fundamental self-advocacy and health literacy skills to prepare them for long-term housing independence.
- Other duties as required.
Qualifications & Experience
- Degree or diploma in Social Work, Community Mental Health, Psychology, or a related human services discipline.
- Minimum of 2 to 5 years of dedicated case management experience working directly with individuals experiencing homelessness, complex trauma, mental health challenges, or substance use disorders.
- Extensive knowledge of Fraser Valley clinical networks, social service frameworks,
and application pathways for provincial health and welfare benefits.
- Deep understanding of Stó:lō culture, protocols, and the local community structure, combined with an understanding of intergenerational trauma and colonization impacts.
- Valid BC Class 5 Driver’s License and a reliable vehicle (required for accompanying residents to service appointments).
- Clear Criminal Record Check with a Vulnerable Sector Clearance.
- Current First Aid/CPR Certification with Naloxone administration training.
