The Pharmacist provides clinical pharmacy services including drug regimen review, medication reconciliation, pharmacokinetic monitoring, individual and group patient medication teaching, drug information and educational programs. Pharmacists work on selected medical services, participate in medical rounds, provide discharge medication calendars and counselling to patients and make therapeutic recommendations to the medical team.
