We are seeking a dedicated Nurse Practitioner to join our team. The ideal candidate will have a passion for working with diverse populations and providing clinical and therapeutic services.Position Requirements:Perform patient health assessment and health maintenance activitiesMonitor the ongoing therapy of patients with mental health illnessDiagnose and treat acute/episodic minor illnessesOffer phone and home consultation/care for acute and episodic illnessesConsistently record and document in the patient’s electronic medical recordDetermine the need for and order all necessary diagnostic imaging, medications, treatments and referralsPrepare and report quarterly data for Ontario HealthEducation:Completion of college/university/CEGEP/vocational or technical trainingCredentials (certificates, licenses,
