You’ll play an key role in follow‑ups, chronic disease management, and same‑day access, while maintaining strong continuity of care under the physician’s direction.ResponsibilitiesConduct comprehensive patient assessments, diagnoses, treatment planning, and health education within the RN(EC) scope of practice and in accordance with CNO standardsSupport the ongoing care of patients from an established physician’s roster, helping maintain continuity and strong long‑term patient relationshipsManage routine primary care visits, follow‑ups, preventative care, and chronic disease management for patients across the lifespanSupport same‑day and urgent patient needs, including episodic care and physician coverage as neededOrder, interpret, and follow up on diagnostic tests, screenings, and investigations within scopeProvide patient counseling and education on treatment plans, medications, preventative care, lifestyle modifications, and community resourcesDeliver compassionate, patient‑centered, and culturally responsive care tailored to each individual’s needs and circumstancesCollaborate closely with the physician and clinic team to coordinate care, ensure appropriate follow‑up, and support high‑quality patient outcomesMaintain accurate and timely clinical documentation within the OSCAR EMR systemContribute to a positive, supportive, and team‑oriented clinic environment focused on excellent patient careAssist with preventative care initiatives,
