Summary of DutiesAs a member of the Assertive Community Treatment Team, the incumbent will provide client centred treatment, rehabilitation, support, and case management to clients in the community with severe & persistent mental illness.Qualifications/SkillsMSW, B.Sc. OT, M. Sc. OT, or COAT approved Equivalent requiredMember in positive standing of the Ontario College of SW and SSW, or College of Occupational Therapist of Ontario requiredRecent community experience workingwith patients with severe and persistent Mental IllnessKnowledge and comfort with functioning within the ACT model of CareBasic knowledge of psychotropic medicationsAbility to work independently and interdependently within a multidisciplinary teamExcellent oral and written communication skillsDemonstrated effective interpersonal skillsDemonstrated problem-solving skillsCommitment to client-centered careKnowledge of the health care system and community resourcesValid Ontario Driver's License requiredAbility to travel extensively within the catchment area
