The Terraces Assisted Living provides primary care to the Terraces residents who reside in individual suites within the building. In this role you will support the assessment and development of the resident care plans.



You will work collaboratively with the multidisciplinary team and families to support resident transitions in care as well as health centre coverage as required.Baycrest Terraces is also connected to The Kimel Family Centre for Brain Health and Wellness, which provide research-based activities to promote brain health and aging using a wide range of recreational, fitness, social and educational programs.Key ResponsibilitiesCommunity development and education, and individual counsellingProvides emergency and urgent care to clients as neededCares for minor episodic illnessMonitors and educates clients with stabilized chronic and long-term illnessQualificationsCurrent registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is requiredCurrent Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is requiredMinimum two (2) years’ experience in gerontological nursing is requiredMinimum five (5) years’ experience in both, Primary Care Community Nursing and acute care is requiredDemonstrated expertise in clinical assessment of seniors with complex medical, cognitive and social needsExcellent interpersonal, communication and problem solving skillsExceptional client service skillsBasic computer skills – Meditech, Microsoft Word and Excel preferred#J-18808-Ljbffr
