Establishes a therapeutic relationship andcreates a detailed plan of care with the patient and family, mutuallydetermining the aspects of care which are most significant. Evaluates the effectiveness of plans andinterventions to promote optimum achievement of established goals. Initiatesand maintains open communication with other members of the team to assure thatthe patient and family receive the full scope of interdisciplinary expertiseand services with a coordinated and integrated plan of care. Communicates with patients, families,subordinate staff and peers to ensure that plans and associated interventionsare understood, operationalized and revised, as appropriate.
