University Health Network (UHN) seeks a Nurse Practitioner to join the Gattuso Centre for Social Medicine and Population Health in Toronto. The role focuses on addressing social determinants of health for patients with unmet clinical and social needs in hospital and community settings.

You will work autonomously as MRP, develop care plans, collaborate with a multidisciplinary team, and lead QI and research initiatives while advocating for marginalized populations.
