Registered Nurses and Practical Nurses – Markham CommunityWe are hiring Registered Nurses and Practical Nurses to work within the Markham community.Pay Per VisitA competitive rate that is paid per client visit, rather than care code or care timeFor longer visits or extra visits (excess of 1hr 59 min), you will be paid an additional visitThis model allows you to work ahead in your day but also take the time you need, as you see fitThis model relies on you using your professional judgment for client careFT status nurses will be guaranteed 70 visits biweekly, allowing for the peace of mind of a guaranteed incomeResponsibilitiesComplete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needsWhen designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.Support the client through care transitions by collaborating and connecting the client with community support services in their local community.RequirementsCurrent registration with the College of Nurses of OntarioCanadian Nurses Association (CNA) specialty certification an assetEnthusiasm and love of community care and passion for evidence-informed practice & excellence in careA current CPR certificate is requiredA valid driver’s license and vehicle (travel required)Updated immunization records, clear criminal background check,
