Key Responsibilities:
• Promote meaningful engagement and participation for IAS
• Implement and coordinate individualized service plans
• Maintain high-quality documentation and service follow-through




• Supervise assigned workers to build service capacity
• Collaborate with external providers and service systems

Requirements:
• Two-year diploma in Human Services or related field
• 3-5 years of experience in disability or community services
• Experience developing individualized support plans
• Proficient in Microsoft 365 and electronic documentation systems
• Clear Vulnerable Sector Check required
