We are seeking a dedicated RPN to play a vital role in our Hospital Partnership Program for home health services. This part time position involves working morning shifts and participating in a bi-weekly weekend rotation. You’ll conduct thorough assessments and provide essential care, including wound management and diabetes support, tailored to each client’s needs.

Key Responsibilities:
• Assess patient needs and plan care accordingly
• Monitor and evaluate care through ongoing assessments
• Ensure high-quality patient care delivery




• Maintain professional relationships with clients and families
• Document patient interactions and care modifications

Requirements:
• Active registration with the College of Nurses of Ontario
• RPN diploma from an accredited institution
• Valid First Aid or CPR Certification
• Experience in acute care nursing preferred
• Reliable vehicle for home visits required
