OverviewWe are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in our Assisted Living Community. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively with diverse patient populations, including seniors, and individuals with dementia or palliative needs. The LPN will play a vital role in supporting patient recovery, managing medication administration, and ensuring a secure and comfortable environment for all patients.ResponsibilitiesAdminister medications and treatments accurately according to physician ordersMonitor patient vital signs and observe for changes in conditionAssist with patient assessments and document findings in EMRProvide wound care, including dressing changes and wound assessmentsSupport patients with activities of daily living (ADLs) and mobility assistanceCollaborate with interdisciplinary teams to develop and implement individualized care plansEducate patients and families about health conditions,



medications, and care instructionsMaintain a clean, safe, and organized environment adhering to infection control protocolsDocument all patient interactions accurately in compliance with healthcare regulationsQualificationsValid Licensed Practical Nurse (LPN) license in good standing with CLHAPrior experience in nursing home, dementia care, home care, hospice care, or senior care preferredKnowledge of medical terminology and medication administration protocolsExperience with electronic health records systems such as Epic or Cerner is advantageousStrong communication skills and compassionate bedside mannerAbility to work effectively both independently and as part of a multidisciplinary teamJoin our team to make a meaningful difference in patients’ lives through dedicated nursing care.
