In this role, you will engage clients through phone and in-person interactions, developing therapeutic relationships that promote their commitment to treatment plans. Key activities include employing health screening tools and providing short-term transitional support to individuals in need. Key Responsibilities:

- Establish therapeutic rapport with clients via calls and face-to-face
- Provide transitional support and case management as necessary

- Diploma in Human Services Counseling or related field
