Unregulated Care Provider (UCP): CareKW is looking for experienced, skilled and reliable individuals to join our team as a UCP to provide support in Facility and Community settings. CareKW is focused on creating a team of qualified individuals to enhance and advance the health of the community. We are open to UCP's with a variety of backgrounds and training expertise.

Shift Details: Employment Type: Full time Shift timing: 8 hour - 12 hour rotational shifts between 6AM and 11PM Schedule:10 shifts biweekly, every other weekend required Competitive starting wages Continuing education and training programs Health care Spending Account Group Retirement Savings Program' Complete regular caregiver skill evaluations, address concerns as appropriate in collaboration with nursing team. Address client or caregiver queries or concerns related to the provision of care (in person or by telephone)



Create and maintain client care plans based on client's identified care needs; communicate changes in client's health status or discrepancies in assigned care tasks to community care coordinator Be versatile in covering open shifts and display willingness to adjust work schedule to ensure coverage in the community Assist with policy creation as needed Provide client-centered, family-centered care Must be available for rotating shifts. Previous experience in providing person care Preferred post-secondary education in related care fields of PSW/HCA/ DSW certification Willing to commute to Kitchener once per week Valid work/study permit (if applicable) Current CPR/First Aid Certification Up-to-date Immunization Records (MMR, TDAP, Varicella, Hep B) Gentle Persuasive Approach (GPA) considered an asset Non-Violence Crisis Intervention (NVCI) considered an asset Ability to follow client care plan CareKW is committed to a culture of equity, inclusion, diversity and anti-racism.
