Employment Type: Part-time in office, on-call for field visits, with potential to move into full time in office.

Start: As soon as possible.
Wage: Starting at $30 per hour, based on nursing designation and relevant experience with additional scheduled on-call compensation and mileage reimbursement for field visits.

Sandra’s Home Health Care Services Inc. is seeking an organized, compassionate and approachable Registered Nurse (RN) or Registered Practical Nurse (RPN) to support our clients, Care Practitioners/PSWs, and office team. This role combines in-office support with field-based responsibilities.

Based at our office inside the Marotta Family Hospital in St. Catharines, this varied position combines client assessments, nursing visits, care planning, staff onboarding and training, and office support.

Role Responsibilities:

- Complete client assessments, reassessments and home visits throughout the Niagara Region.
- Develop and update care plans, providing clear directions for staff.
- Provide nursing care within your scope of practice, monitor changes in client condition, and coordinate appropriate follow-up.
- Lead onboarding and orientation for new Care Practitioners/PSWs, including care procedures, documentation, communication expectations and AlayaCare.
- Provide hands-on and client-specific training, assess staff competency within your nursing scope, and deliver ongoing coaching and refresher training.
- Review care documentation and follow up on concerns with staff, clients and families.
- Communicate with healthcare providers to support coordinated care.
- Assist with care coordination,



service planning and scheduling as needed.
- Participate in scheduled on-call coverage and respond to after-hours concerns.
- Maintain accurate records and contribute to quality improvement and audit preparation.

Requirements:

- Current registration in good standing with the College of Nurses of Ontario as an RN or RPN.
- Strong assessment, communication, documentation and organizational skills.
- Sound clinical judgment and the ability to work independently, prioritize concerns, and seek additional support when needed.
- A patient and supportive approach to teaching and coaching staff.
- Comfort using electronic care records and office software; AlayaCare experience is an asset.
- A valid driver’s licence and reliable vehicle for visits throughout Niagara.
- Availability for office hours, field visits and scheduled on-call coverage.
- Current CPR certification and ability to meet required screening requirements.
- Experience in home care, geriatrics, dementia care, palliative care or staff education is an asset.
