WE'RE HIRING! Community and Residential Lead
HeARTh Social Profit Company
HeARTh Social Profit Company is a small, growing, and dynamic agency, and we are expanding our team!
At HeARTh, we believe in supporting both the people we serve and the people who work alongside us. Our strength-based approach encourages staff to develop their skills, explore their interests, take initiative, and grow into their potential.
Our niche is change. We recognize that supporting individuals with complex needs requires creativity, flexibility, collaboration, and a willingness to think outside the box. We don't believe in a one-size-fits-all approach. We believe in finding solutions, building capacity, and helping people live meaningful and fulfilling lives.
We are looking for skilled, creative, reliable, committed, and passionate individuals who want to make a difference and contribute to a strong, supportive team.
The Opportunity: Community and Residential Lead Support Worker
We are seeking a dynamic and experienced individual to take on a leadership role supporting adults funded through Alberta's Persons with Developmental Disabilities (PDD) program.
This is a hands-on leadership position that combines direct participant support, case management, household operations, team leadership, and service planning.
Our participants may experience significant and overlapping needs involving complex mental health, addictions, behavioural challenges, medical conditions, judicial involvement, and other health and support needs.
We are looking for someone who can lead by example, work alongside their team, take ownership of responsibilities, and ensure that plans and expectations are carried through.
Key Responsibilities
Participant Support & Case Management
- Provide individualized support with daily living, life skills, personal care, emotional well-being, appointments, employment, and community participation.
- Support individuals with complex behavioural, mental health, medical, addiction-related, and judicial needs.
- Participate in the development, implementation, and ongoing review of participant goals, support plans,
and strategies.
- Complete accurate daily documentation, monthly reporting, and other required case management responsibilities.
- Promote independence, dignity, choice, safety, and meaningful community inclusion.
House Leadership & Team Support
- Take an active leadership role in the daily operations and organization of a participant home.
- Provide direction, guidance, coaching, and practical support to team members.
- Help ensure staff understand their responsibilities and follow through on established plans, procedures, and expectations.
- Identify challenges, problem-solve, and communicate concerns proactively.
- Maintain organized household operations, documentation, schedules, and routines.
- Work collaboratively with coordinators, management, participants, families, and other professionals.
- Demonstrate accountability, professionalism, consistency, and leadership through action.
Who We're Looking For
We are seeking someone who brings more than experience. We want someone with the confidence, initiative, and commitment to take responsibility, make sound decisions within their role, and contribute to a strong team.
Required Qualifications & Skills
- Experience supporting individuals with complex behaviours and case management responsibilities is essential.
- Experience supporting adults with developmental disabilities and complex mental health needs.
- Strong leadership qualities and the ability to guide and support a team.
- Excellent organizational skills, attention to detail, and follow-through.
- Solid communication, problem-solving, and conflict-resolution skills.
- Ability to work independently while contributing positively to a team.
- Reliability, accountability, adaptability,
and initiative.
- Ability to remain calm, professional, and effective in challenging or changing situations.
- Ability to meet the physical and emotional demands of the position, with reasonable accommodation where applicable.
- Valid driver's licence, access to a reliable vehicle, and a driving record acceptable for work-related transportation.
Assets
- Previous experience leading or managing operations within a supported living home.
- Experience with PDD-funded services and associated documentation and planning.

- Experience mentoring, coaching, or training staff.
Schedule & Flexibility
This position requires flexibility and adaptability to meet participant and operational needs.
Shifts may include weekdays, weekends, evenings, and overnight shifts in participant homes and community settings.
The successful candidate must be comfortable working in a changing environment, balancing competing priorities, and adjusting their schedule based on the needs of their caseload.
