Bilingual Patient Care Coordinator – Full-Time

Status: Full-Time

Location: Ottawa, Ontario (On-Site)

Start Date: ASAP

About Perennial

Perennial delivers integrated primary care and wellness services directly within retirement residences, combining in office and virtual care to meet seniors where they are. By proactively anticipating the needs of older adults, we improve quality of life, reduce unnecessary hospital visits, and ease the strain on the public healthcare system.

Today, we are active in over 30 retirement residences across Ontario, Saskatchewan, Alberta, Manitoba and Quebec with continued plans for growth and expansion.

Our interdisciplinary team including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes.

About The Role

The Patient Care Coordinator (PCC) is Perennial's person in the building. You are in our Ottawa homes on the days our NP is there, keeping the clinic running, knowing every resident, and making sure the people who need allied health get it. You are the face residents and staff recognise and the first call for anything Perennial.

This is a new role and you will be the first on-site PCC in the Ottawa region. You will cover three to four retirement communities in the city. The role exists because care for seniors depends on familiar faces and follow-through, and that cannot be done by phone.

What you will do

Support the NP and the clinic

- Prepare each clinic day: rooms ready, residents roomed with vitals, schedule full
- Book NP follow-ups and any allied health the NP recommends before the resident leaves the room
- Clear NP admin the same day: forms, faxes, family callbacks, referral paperwork
- Support virtual and phone appointments: get the resident to the room, set up the device, stay with them

Run allied health in your homes

- Own the relationship with the allied health providers who visit your three homes; confirm each session 48 hours out and keep every session full
- Follow up with every resident the NP saw that week:

a wellness check, and a booking for any of our six services they need
- Call or visit the resident or POA after every allied health appointment and rebook the next cycle
- Walk residents to their appointments and coordinate the day when a provider is in the building

Be the home's single point of contact

- Five-minute check-in with the Executive Director or Director of Care at the start of every shift: new move-ins, falls, hospital returns, family concerns
- Handle anything Perennial-related the home raises, the same day
- Monthly 15-minute review with the Director of Care on residents who should be on allied health and are not
- Let the ED know if other providers are soliciting in the home

Know every resident

- Welcome every new resident within 7 days of move-in with a Perennial package; capture their preferences, current providers and POA details
- Keep the resident roster current in our EMR
- Work the check-in sign-up sheet at your desk first thing every shift
- Eat lunch in the dining room with residents, not in the office
- Host a regular open hour (coffee, ice cream, ask the care team) in a common area

Be Perennial in the building

- Perennial shirt and name badge every shift, at a desk by reception or the busiest common area
- Keep posters, sign-up sheets and provider date cards posted and current; monthly signage walk with photos
- Run one activation a month: a hearing day, foot care open house, flu clinic support or similar

Who we are looking for

You like being on the floor with seniors, you are organised enough to run a clinic day from a checklist, and you are comfortable being the only Perennial person in the building.

- Experience in a patient-facing healthcare role is an asset: medical office assistant, ward clerk, PSW, activation or recreation aide,

or clinic receptionist
- Comfortable with an EMR and a booking calendar; you log things the same day, every day
- Warm and visible in person; residents and staff remember your name after one shift
- Calm with families and POAs on the phone; clear and respectful in difficult conversations
- Reliable on a fixed schedule; the role only works if you are in the same home on the same days every week
- Able to travel between 2-4 Ottawa homes; a vehicle is strongly preferred
- Bilingual English and French is a strong asset in Ottawa and may be required for some homes
- Nice to have: experience in retirement or long-term care

This is not a sales job. We track allied health bookings because they tell us residents are getting care they need, and the way you get them is by being trusted, present, and useful to the NP and the home.

Details

- Full-time, 40 hours per week, Monday to Friday, fixed days per home
- On site in 2-4 Ottawa retirement communities; no remote work
- Reports to the Manager, Clinic Operations and Growth; day-to-day support from the central Patient Care Coordinator
- Mileage between homes reimbursed
- Vulnerable sector check and immunization records required before start
- Start date: as soon as possible
