WNHAC is seeking a Primary Care Nurse/ Care Coordinator to join our outreach clinic. You will provide nursing services in diverse settings serving homeless and substance use-impacted populations, following Ontario nursing standards.

You will collaborate with care teams, perform assessments, and support care coordination to ensure timely diagnostics, screenings, and patient education in a trauma-informed, culturally respectful setting.
