RENFREW VICTORIA HOSPITAL
Personal Support Worker – ALIGN Program
Part-Time
Date: October 2026
Posting Number: 26-88
The Renfrew Victoria Hospital (RVH) is a 55-bed, fully accredited facility located just 45 minutes west of Ottawa, serving a catchment area of over 60,000. Part of the Renfrew and Area Health Village, RVH provides medical, surgical and complex continuing care as well as extensive ambulatory care with over 30,000 emergency and clinic visits per year. RVH has recently launched a new innovative ALIGN (Aging at Home through Linked, Integrated, and Guided Networked Care) Program.
The ALIGN Program (Aging at Home through Linked, Integrated, and Guided Networked Care), launching in September 2026 at Renfrew Victoria Hospital (RVH) and St. Francis Memorial Hospital (SFMH), is a team-oriented model of care to wrap supports around patients who are ready to be discharged to home.
ALIGN brings together multidisciplinary care teams and shared clinical leadership across both organizations to identify patient needs early, coordinate care planning, and ensure patients receive the right care at the right time and in the most appropriate setting. By connecting patients more seamlessly to rehabilitation, transitional care, community supports, and other services, the program helps reduce fragmentation of care and creates a more coordinated, patient-centred experience for patients and families.
Through shared expertise, standardized processes, and integrated planning, ALIGN is designed to improve patient flow, support timely transitions from hospital to home or community-based care, and enhance outcomes for patients with complex needs. Key measures of success include reducing Alternate Level of Care (ALC) days, shortening hospital lengths of stay, and improving the timeliness of care transitions.
For rural communities, ALIGN highlights how RVH and SFMH can leverage their integrated model to maximize scarce healthcare resources, strengthen collaboration among providers, and deliver high-quality care close to home while improving efficiency and sustainability across the healthcare system.
