Shared Health in Winnipeg seeks a Nurse Service Coordinator for the Adaptable ACT team (FACTT/ACT). You will provide case management, coordinate medical services and medication administration, and deliver crisis-on-call support within a multidisciplinary team.

The role requires a nursing degree and Manitoba registration, with four years of relevant community experience and a valid driver’s license. The position is in-person with shift coverage in days/evenings.
