Liquid Personnel is recruiting a Social Worker for its Transition to Adulthood Team in Cornwall. You will support young people moving from Children’s Services into Adult Social Care, using a strengths-based, person-centred approach.

The role requires experience in transition work, solid assessment skills, and knowledge of Care Act 2014 and related legislation. Cooperative, rights-based planning is essential.
