Serve the community as a Licensed Practical Nurse with The Salvation Army. Focus on patient care and support in a residential setting while working closely with health care teams. We are looking for a Licensed Practical Nurse to join our team and deliver exceptional care at Grace Manor or Supported Housing.

This role requires solid assessment skills, familiarity with mental health care, and the ability to collaborate with interdisciplinary teams. Successful candidates will engage in patient evaluations, care planning, and implementation to ensure high-quality resident support. Key Responsibilities:
Conduct ongoing nursing assessments of assigned residents
Collaborate on individualized care plans based on resident needs




Implement evidence-based nursing interventions and treatments
Monitor and document changes in resident health status
Communicate effectively with healthcare team members Requirements:
Graduate of an approved Licensed Practical Nurse Program
Active registration with CLPNA required
Minimum two years of experience in continuing care preferred
Basic Life Support certification necessary
